The high-frequency electrosurgical scalpel, commonly known as a high-frequency (HF) electrosurgical knife, is a powerful tool widely used in modern surgical procedures. It works by generating high-frequency, high-voltage currents through the main unit's oscillating circuits. These currents are delivered to the surgical site via the scalpel tip, causing an instantaneous cauterization effect on the contacted tissue. This allows surgeons to perform either cutting (electrotomy) or coagulation (electrocoagulation) with precision.

The difference between cutting and coagulation lies in the waveform of the high-frequency current:

  • Cutting (Electrotomy): Uses a continuous high-frequency waveform. The instantaneous energy generates a large amount of heat, producing extremely high temperatures that vaporize cells and separate tissue. Cutting requires relatively low voltage but delivers densely concentrated energy.

  • Coagulation (Electrocoagulation): Uses an intermittent, pulsed waveform. The heat generated is lower and insufficient to vaporize tissue, resulting in a controlled burning effect that seals blood vessels. Coagulation has lower energy but higher voltage.

  • Blend Mode: A hybrid waveform that falls between cutting and coagulation, offering a balance of tissue dissection and hemostasis.

For the HF current to act safely on the human body, it must complete a closed circuit by returning to the electrosurgical unit. In monopolar mode, the current flows back through a grounding pad placed on the patient. In bipolar mode, such as with bipolar forceps, one tip delivers the current while the other returns it, eliminating the need for a separate grounding pad.

Advantages of High-Frequency Electrosurgical Scalpels:

  1. Efficient and Precise: Provides fast cutting, effective hemostasis, clean incisions, and simple operation, ensuring safety and convenience.

  2. Time-Saving and Reduces Blood Loss: Compared to traditional mechanical scalpels, HF electrosurgical devices can significantly shorten surgical time, reduce blood loss and transfusion requirements, and thereby lower complications and overall costs.

  3. Versatile and Cost-Effective: Compared to other electrosurgical devices such as lasers, microwave knives, ultrasonic scalpels, water jets, or semiconductor coagulators, HF electrosurgical knives cover a wider range of surgical applications, are easy to maneuver, and offer a high cost-performance ratio for both the main unit and consumables.

High-frequency electrosurgery continues to be an indispensable technology in modern operating rooms, improving surgical efficiency, patient safety, and clinical outcomes.
